Based on these question type, I do not think forms can break out the address the way you mentioned, it would need separate questions to collect Street, Apartment etc. If my understanding above is not consistent with yours, you can also post back and point that out. If you click New to compose an email, your Gmail address and server will be used to send the message. Any replies will go to the Gmail server. The only way emails can be received is by adding an @outlook.com alias address to your account. What you may see now is your Gmail account is setup as a connected account.
