    The credit shows up on the HUD statement. The W9 means it is income - and subject to taxes. The W9 will just have your info on it, along with your social security number (since you don't have a business ID #).     The form can be printed off the IRS website pretty easily. Probably has to do with the amount being greater than $700. Yes, but then the OP is going to have to pay income tax on the cost of the travel reimbursement because the company is going to report her to the tax authorities as an independent contractor with a 1099 Misc. form.

    The bank sent me a paper form called a Signature Form, along with a receipt and a list of interest rates (which I already could see on the website). I was supposed to sign it and return it. It said something about "W9" on it. Was this just to give the bank an image of my signature? I was annoyed they didn't exactly spell out why they sent me this.

    I was there six months and he issued a 1099 for the work I performed. HireRight couldn't understand that I was self-employed (it "doesn't compute" with them as people aren't running a background investigation, the company is simply doing an automated report assembly off a form you fill out both in writing and online).
