Funds cannot be extracted from cards. If you want to use the card, select the card at checkout and make sure the amount on card covers the entire purchase or it will decline. When I checked the balance of my card on the american express website I saw the item I purchased and the amount paid as well as a mysterious $1 transaction. They were separate transactions.
I tried setting up an American Express gift card with a remaining balance of $12.61, trying to purchase a $12.00 item from offworld percussion. without even confirming, paypal billed me 1.95 as a confirmation fee, yet it is a gift card therefore i have no way of confirming the card and getting my $1.95 back, and now i only have $9.66.

If you paid using your PayPal Account Balance or Bank Account, the refund is credited back to your PayPal Account Balance. Times can vary for many reasons, it can take from 1 day to several days depending on the funding source used by the party doing the refund. If you paid using a credit card, the refund is credited back to that card.
That being said, if you would like to attempt to add and use the card, you need only complete the registration process successfully for the card (linking and confirming the card may be required), and then select it as a funding source at checkout. PayPal will debit the card directly - there won't be a need to add funds into your balance first!
